This is not a clinical afterthought. A landmark systematic review and network meta-analysis published in The Lancet Psychiatry in December 2025 — the largest and most rigorous analysis of antidepressant deprescribing strategies to date, examining 76 randomised controlled trials — found that gradual tapering of antidepressants with psychological support is as effective as remaining on medication for preventing relapse. The review also found that psychological support improved outcomes across all tapering strategies, and — critically — that slow tapering without psychological support was no better than abrupt discontinuation.
